The NHS provides sickness certification only after seven continuous days of illness. For shorter absences, employees use a self-certification form (SC2). After seven days, a GP may issue an NHS Fit Note — but you will need to contact the surgery to discuss your request, as an appointment with your GP may be required.

If your illness has lasted more than seven continuous days, your GP can issue an NHS Fit Note (also called a Med 3 or "sick note"). This may be issued after a consultation — please contact the surgery to arrange this.
No — you do not need to see your GP to be 'signed back to work'. Once your period of sickness has ended and you feel well enough to return, you can simply go back. There is no requirement for a GP to issue any documentation confirming your fitness to return. Your employer cannot insist on one from your GP as a condition of return.

No. NHS Fit Notes (Med 3 certificates) are provided free of charge for patients who have been ill for more than seven days. There is no fee for this service when issued by your GP practice.
They are the same document. The NHS Fit Note (formally "Med 3") replaced the older "sick note" in 2010. As well as confirming incapacity for work, a Fit Note can also indicate whether you may be fit for work with adjustments — such as reduced hours or altered duties — helping employers support a phased return.
In some circumstances a Fit Note can be backdated to cover the full period of illness. However, GPs use their clinical judgement and are not obliged to backdate. The best approach is to contact the surgery as soon as possible during your illness rather than waiting until you have recovered.
Employers can ask for proof of illness for shorter periods, but they cannot require a GP's note — only a self-certification form (SC2). If your employer is asking for a GP note for an absence of 7 days or fewer, you can advise them this is not a requirement under NHS guidance and provide a completed SC2 form instead.
