Hair Loss — Understanding the Cause

Male-pattern baldness (androgenetic alopecia) is the most common type of hair loss in men, affecting around 6.5 million men in the UK. It is caused by hair follicles gradually shrinking in response to dihydrotestosterone (DHT) — a hormone derived from testosterone.

  • Typically begins with thinning at the temples or crown
  • Follows a recognisable pattern (the Norwood scale)
  • Can begin as early as the late teens; most men have some loss by 60
  • Caused by genetic sensitivity of follicles to DHT
  • Hair follicles gradually miniaturise rather than dying

💊 Treatment Options

Several treatments can slow or partially reverse hair loss. Speak to your GP to discuss what may suit you:

  • Minoxidil — topical solution applied to the scalp; available over the counter
  • Finasteride — prescription tablet that blocks DHT; effective for many men
  • Hair transplant surgery for those who prefer a permanent option
  • Psychological support if hair loss is affecting mood or self-esteem

Information and services for sexually transmitted infections (STIs), HIV, and blood-borne viruses are available through North West London Sexual Health Services.

  • Free, confidential STI testing for all men regardless of age
  • HIV testing and pre-exposure prophylaxis (PrEP) referrals
  • Treatment and follow-up for diagnosed infections
  • Referral to specialist sexual health clinics when needed

Problems with ejaculation or erections are common and treatable. Your GP can discuss these concerns in a non-judgemental way and recommend appropriate management options, including lifestyle advice, medications, or referral to a specialist.

Prostate Cancer — Know the Facts

Prostate cancer is the most common cancer in UK men, with over 40,000 new cases each year. It usually develops slowly, and many men live for years without any symptoms. The prostate — roughly the size of a satsuma — surrounds the urethra, so enlargement eventually affects urination.

  • Increased frequency or urgency to urinate, especially at night
  • Difficulty starting urination, or a weak/interrupted stream
  • Feeling that the bladder has not fully emptied
  • Blood in urine or semen (less common)
  • These symptoms are more often caused by BPH (benign enlargement) — but should always be checked

🩺 PSA Testing

The PSA (prostate-specific antigen) blood test can help identify men who may need further investigation. It is not a definitive cancer test, but an elevated result prompts referral for further assessment.

  • Men aged 50+ can request a PSA test at their GP surgery
  • Higher-risk groups (Black men, family history) may discuss earlier testing
  • A raised PSA does not confirm cancer — but warrants investigation
  • Digital rectal examination (DRE) may be performed alongside the blood test

Do not ignore urinary symptoms. Book an appointment with your GP — most causes are benign and easily managed, but early assessment is important.
Testicular Cancer

Testicular Cancer — Check Regularly

Testicular cancer is the most common cancer in men aged 15–45. Though relatively rare overall, it is highly curable when caught early — with a 98% cure rate at the earliest stage, and 96% survival at 10 years even for advanced disease. Regular self-examination is key.

  • A change in size, shape, or feel of a testicle
  • A firm, smooth, slow-growing lump — often initially painless
  • Swelling or thickening of a testicle
  • A feeling of heaviness in the scrotum
  • A dull ache in the lower abdomen or groin

👐 How to Self-Examine

The NHS recommends checking your testicles regularly — ideally after a warm bath or shower when the scrotum is relaxed:

  • Hold each testicle gently between thumb and fingers and roll it slowly
  • Feel for any hard lumps, changes in shape, or differences in size
  • It is normal for one testicle to be slightly larger or hang lower
  • The epididymis (soft, rope-like structure at the back) is normal — do not confuse it with a lump
  • Visit your GP immediately if you notice any change

If you notice any lump or swelling on a testicle, see your GP as soon as possible. Early diagnosis makes an enormous difference to treatment and recovery.
