Confirm you live within our catchment area — primarily UB1, UB2 and surrounding parts of West London. Call us if you're unsure.
Register online via NHS.uk, download and return the GMS1 paper form, or visit us in person. Bring proof of address and ID where possible.
Book a short health check with our nursing team. This helps us gather key health information while we wait for your medical records to arrive.
Once your registration medical is done, you can book appointments with our GPs online, by phone, or via the NHS App.

We register patients who live within our catchment area, which covers Southall and surrounding parts of West London — primarily postcodes UB1 and UB2.

When visiting us in person to register, it helps to bring the following:
Once registered, we invite all new patients to book a registration medical with one of our practice nurses or healthcare assistants. This is an important appointment that enables us to collect key medical information before your full records arrive from your previous GP — a process that can take several weeks.
During the appointment we will take a brief medical history, check baseline measurements (blood pressure, weight, height), review any current medications, and begin screening for other medical needs. You will normally need to complete this before seeing one of our GPs for a routine appointment.
Not registering as a permanent patient? We can still help.
If you are not registered with a GP but need to be seen urgently, you can receive emergency treatment for up to 14 days without formally registering. After that, you must register as either a temporary or permanent patient.
You can register as a temporary resident if you are in the area for longer than 24 hours but less than three months. Please complete a Temporary Resident Form (GMS3) at reception and return it to us.
NHS GP services are available to all people in England regardless of nationality or immigration status. However, some services may be chargeable for overseas visitors not ordinarily resident in the UK.
